you fill out the forms to the best of your ability. If you do not know an answer please leave it blank (do not guess).
You must save the Personal Profile to
your computer before you attach it to an
email.
We require a
signed copy of the Authorization in
order to process your application.
You may print out the Authorization,
sign it and scan it to return by email.
You may also submit the form via fax or
regular mail. Please use the fax
number and address listed on our contact
page. We can not process an
application without a signed
Authorization as it is the only proof we
have that an applicant has given us
permission to share their information
with the intended parent(s).

Email your completed forms along with 2-4 photographs to Donors@elitedonors.com.
Please make sure the photos are of a
high enough resolution that they can be
printed out in a 3x5 size.
Please submit at least one full length photo and one close-up/headshot.
Make sure you are fully clothed in your photographs.
Please refrain from sending photos where
your face is obscured or where you are
wearing a hat or sunglasses.

to numerous inquiries about the ad.
It does not mean that the intended
parents are not happy with the
applications they have received to date.
They would rather all interested parties
had an opportunity to thoughtfully fill
out the application instead of
submitting an incomplete application.
